Registration Information

The cost to participate (registration fee) in the 2008 SMYA Football or Cheerleader Program is $150.00
for Football and $175.00 for Cheerleading.  Registration fees can be paid online through *PayPal or by
personal check (with Driver’s License), money order, and cash.  All fees must be paid before the child
can receive any football equipment, cheerleader uniforms or participate in any practices or games.  
Because we feel it is our obligation to keep our parents informed, as well as, your right to know, we
have compiled a list of what is included in the registration fee.

~~Special Notes~~

This year for every child registered the parents will receive $50.00 worth of raffle tickets (10 tickets
worth $5.00 each).  The raffle tickets are nonrefundable.  Parents have the option to sell the tickets and
get up to $50.00 of their registration cost back or keep the tickets and remain in the drawing to win the
prizes being given away on SMYA Homecoming Saturday.    

The Football Bundle Package is something new this year.  Each participant will purchase and receive his
own practice pants with pads, and practice jersey.  These items will belong to the child.  Therefore, if the
child can use them again the following year your registration cost for 2009 will be minus the Football
Bundle Package charge of $25.00.

Early Registration Form.

** Must be returned at the conclusion of the season or when the child no longer participates with his/her
team during the season.

Instructions For Registration

1.  Fill out the Form Below, then click submit,
then click the back button.

2.  Click on the appropriate Buy Now Button for payment.
Please note that the prices for 2 or more children have been calculated for you. Also, in order to
receive the registration discount, All children must live in the same household, which is subject to
verification.

The calculations for Paypal are spelled out in the example below.  For multiple siblings household
families (2 or more) a $25.00 discounts will be applied for a family registering 2 kids and an additional
$25.00 for a family registering of 3.  The maximum total discount is $50.00.  If a family registers more
the 3 children the regular registration rate will apply towards any kids after 3.  

Examples:
a.        A family of 2 kids (both football) will pay $150.00 + $125.00 = $275.00
b.        A family of 2 kids (1 football & 1 cheerleading) will pay $150.00 + $150.00 = $300.00
c.        A family of 3 kids (2 football & 1 cheerleading) will pay $150.00 + $125.00 + $150.00 = $425.00
d.        A family of 4 kids (2 football & 2 cheerleading) will pay $125.00 + $125.00 + $175.00 + $175.00 = $600.00

Refund Policy:  
If a parent chooses to remove their child
from the SMYA Football and/or
Cheerleading Program during that child’
s 1st week of practice, parents will
receive a registration refund of only
$60.00 for football and $80.00 for
cheerleading.  When a parent initially
registers they are provided with $40.00
worth of raffle tickets.  The raffle tickets
are nonrefundable.  However, parents
still have the option to sell the tickets and
get an additional $40.00 to add to their
refund amount or keep the tickets and
remain in the drawing to win the prizes
being given away.  The football and
cheerleading refund period ends
August 4, 2008 at 8:30 p.m.
